An accessible channel for stakeholders and third parties
The area allows clients, suppliers, staff and stakeholders to submit reports or claims concerning services, conduct, processes or documentation.
Those who wish to remain anonymous may omit name, email and any other identifying information from the message or form.

Email form and DOC 63 template
The form prepares a communication addressed directly and exclusively to the Supervisory Body (ODV), with the essential information needed to classify and handle the report.
Alternatively, the DOC 63 template can be downloaded, completed and sent to the ODV through the channel indicated in the procedure.
Identifying details are optional
Personal information is requested only if the reporting person wishes to be contacted for updates or further details.
Received communications are handled with attention to confidentiality and personal data protection.
